Integrated community care for people with mental challenges

Temasek Cares - iCommunity@North is a pilot programme that provides community-based integrated social and healthcare services to people with mental challenges. The programme enables clients to remain in the community, with less reliance on hospital or in-patient care. The iCommunity@North programme is expected to serve 300 clients over two years.
Mental conditions such as dementia and depression are becoming more common as our population ages (see Chart 1). To address this growing need, the Agency for Integrated Care (AIC) has put together a new mental health network that provides integrated and person-centred care for people with mental challenges.
This network comprises multiple agencies from social services, healthcare and the community. They provide services through existing support systems including senior activity centres, family service centres, wellness centres, general practitioners and hospitals. The services cover the care continuum from education, prevention, early identification, intervention, and follow-up to home-based support
Implementation of the new mental health integrated network is expected to take five years. The first phase is supported by Temasek Cares and implemented by AIC in partnership with Khoo Teck Puat Hospital, Thye Hua Kwan Moral Society and O’Joy Care Services.
Temasek Cares - iCommunity@North comprises two intervention and outreach programmes: COMIT (Community Mental Health Intervention Team) and CREST (Community Outreach and Support Team).
COMIT provides counselling and therapy support in the community and is led by allied health professionals including nurses, counsellors and medical social workers. The team serves patients with mild with to moderate mental challenges who are assessed to be capable of living in the community with adequate some intervention. CREST provides outreach, education and emotional support to clients and caregivers. The programme plays a key role in prevention and early detection.
