Ageing in Place @ Tanjong Rhu
Providing home-visit and rehabilitation services to the frail elderly in Tanjong Rhu
Partner: St Hilda's Community Services Centre

This is a pilot programme that provides community-based services to assist the elderly residents age in place through the provision of a neighbourhood rehabilitation centre and a home visit service by healthcare professionals.

There will be a new rehabilitation centre to cater to many elderly residents that require rehabilitation services for age-related conditions. Many of the elderly do not go for scheduled therapy sessions because of financial constraints, inconvenience, lack of caregiver support and lack of awareness. As a result, their physical functioning ability is compromised.

There is also a home visit programme to provide medical, nursing, therapy and hospice services to the frail elderly who are homebound or terminally ill. The team of doctors and nurses will visit the frail elderly in their homes regularly. In addition to providing medical and nursing care, support and advice will be given to caregivers on how to take care of the elderly. Therapy and palliative home visits will be provided when needed.

The pilot programme hopes to offer affordable and convenient home-visit and rehabilitation services to the frail elderly in order for them to age in place with independence and dignity. If the programme is successful it can be duplicated in other estates.
