Temasek Cares – SPICE
Providing integrated and community-based services to the frail elderly

Singapore Programme for Integrated Care for the Elderly (SPICE) provides integrated and community-based medical, nursing and social services for the frail elderly. The programme caters to elderly patients who have functional impairments coupled with multiple medical co-morbidities. Although they are not bedridden, they require a high level of medical and nursing care usually provided in a nursing home. By providing such services within the community, SPICE enables the elderly to age at home surrounded by their loved ones.

SPICE is modeled after an adult day health centre concept that has been proven to be successful overseas. Locally, an existing day rehabilitation centre is modified and enhanced to run the programme. A multi-disciplinary team comprising nurses, therapists, case managers and health workers looks after the care needs of the elderly. This team works closely with the patient’s primary physician, who may be a General Practitioner, polyclinic doctor or a specialist in the hospitals. The case manager works with the various providers to ensure delivery of the individualised care plan and helps to coordinate services that are not available in-house.

With the enhanced services, improved facilities and extended hours (7am – 7pm), the frail elderly are able to receive needed medical, nursing and social care within the community during the day and still be able to stay at home with their families and loved ones during the evenings and on weekends. Caregivers can continue to work and get respite from caring for the elderly during the day.

SPICE is set up as a proof of concept programme. If the 1-year pilot is successful, it can pave the way for future programmes that transform the way society cares for its elderly.
